Liability Waiver

I, a parent/ legal guardian agree (on behalf of myself, my children, my heirs, executors, administrators, and assigns) to release, discharge, waive and relinquish F.U.N. Mothers’ Club (including but not limited to its board, officers, agents, members, coordinators, volunteers) from any and all liabilities, claims, or actions for loss, personal injury, property damage, or wrongful death connected with our participation in club activities, meetings, socials and events both indoors and outdoors. I understand and acknowledge that F.U.N. Mothers’ Club does not carry or maintain health, medical, disability or general liability insurance coverage and therefore I agree to assume responsibility for such insurance coverage for me and my family. I will be responsible for the safety and behavior of my family and guests at all club events and functions.
